I used this one...not original by any stretch but the players enjoyed it.
So they aquired an anciant map that shows the location of a church that a legendary Paladin was rumored to be buried. (In my world there is a separate small continent inhabited by Orcs where it is located)
Once the party reached the area it is just a giant field of boulders of all shapes and sizes. Upon inspecting the boulders which are randomly spread about they will discover carvings...some recent and others worn with age.
The puzzle is actually pretty simple (which I find are the best). If you look closely at the cliff the party will see a very large symbol carved into the face of the cliff....it is anciant and worn completely out in some areas...but still hard to miss due to the sheer size of it. Upon closer inspection it looks like there is a hole in the center just big enough for a man to crawl into.
The climb is extremely treacherous... But if a character makes it to the hole he will find it only goes back abot 3 to 5 feet.
It is when he\she turns around that they will see an exact match of the symbol in the arrangement of the boulders...impossible to notice from the ground.
And of course under the center Boulder is the entrance to the dungeon.
Please share cool ways you have led players to dungeon entrances. Even if they are ciiche...ID still like to steal the....err I mean read them.

Mine wasn't leading into a dungeon - mine lead out of the dungeon - the last three rooms.
The players entered a room with a large room that was a ruined temple. Opposite from the entrance was a large set of stone double doors. They did a perception check because they couldn't open the doors. The stone doors were magically sealed. The inscription was a poem that read:
"Those who travel through the dark Carry within their blood the mark. To pass through Heaven's Gate They must have help to deny their fate."
Only good or neutral characters could pass through the doors. One PC was neutral, but he was from the Shadow plane, so he couldn't get through either. The neutral & good players passed through the door and looked around for something to get the other two members through the doors. They found three potions. One was a dark liquid that made anything light into dark for a day. One was green liquid that turned evil to good for a day. One was a white liquid that turned anything shadowed solid for one day.
The green liquid let the evil character through the door and the white liquid let the shadow character through the door.
To exit that room, they had another seal. It read: "To pass through this room, you must go from the sun to the moon." - which meant they all had to drink the dark liquid which made them all evil for one day. So that led to some chaos. They were restored to themselves by drinking from a well per the dragon's instructions in the final (third) room. And the dragon was kind enough to tell them which passage to take to get out.
