Okay, this is one I've used a couple of times before, and it has amused both myself and players who just wanted to see what happens. It's a twist on a regular mechanical spike trap. When the pressure plate is triggered spikes jut out from the floor and ceiling. Nothing special there, really; anyone who falls into gets hit by 1d3 spikes for 1d6 damage each and it can be disarmed by a thief like any other trap.
However, the builder of the trap has overcompensated on the pressure mechanism, and the spikes come out with excessive force, and there is a high chance per spike that hits the victim (I usually do a cumulative 10% per spike) that they are launched off of the floor, which resets the trap, causing the spikes to rapidly retract. The victim then falls back onto the pressure plate and resets the trap, creating a "gnashing teeth effect."
